More so than in any other specialty of medicine, the practice of opthalmology deals constantly
with a priceless precision - sight. This requires mutual understanding and respect between the
physician and patient.
I will strive to offer you the best and highest standards of medical eye care and request you
reciprocate by having confidence in my judgement. If you have any doubts, please convey them
directly to me and don't hesitate to request another ophthalmologist's opinion.
If I feel that surgery is necessary in your case, then I will advise those procedures
that are in your best interest.

The practice of ophthalmology is devoted to the diagnosis and treatment of all eye problems. Your
ophthalmologist is the only person qualified to diagnose and treat all eye problems. This includes
blurred vision, crossed eyes, glaucoma, cataracts, retinal detachments, burns, injuries and any other
eye aliment.
Our highly-trained staff is qualified to assist in your visit. We have made every effort to make this
office one of the most well-equipped and modern facilities in the United States. In fact, we have equipped our
office with two different lasers for various types of eye surgery. We also have numerous other computerized
high-tech pieces of equipment that provide the latest eye examination techniques.

When you call, please explain your needs and problems as accurately as possible to the receptionist.
This will enable her to schedule the right amount of time to your appointment.
If you need to reach someone after hours, call either of our office locations
for instructions. If for some reason you can't reach anyone, go to the emergency
room at either Cape Coral Hospital or East Pointe Hospital.

All appointment are made in advance. If you are unable to keep an appointment, please contact our
office as soon as possible for rescheduling. This allows us to switch another patient to your time
and not cause excessive delay for the future.
Every effort will be made to honor your appointment time. We firmly believe in the value of your time
and dislike long office waiting periods.
Occasionally, emergency patients may cause unforseen delays. Please be patient should this occur.

Once Dr. Croley completely examines your eyes and determines you require glasses, he will issue you a
prescription. You may take this prescription to the optical store of your choice. As an added service
and convenience for our patients, an optical department is located within Cataract & Refractive
Institute of Florida at our Cape Coral and Lehigh locations.

While not all people can wear contact lenses, the fitting procedure is far more advanced now than it
was several years ago. Following your eye exam, Dr. Croley will tell you whether you can wear contact lenses.
Since the lenses are in constant contact with your eyes, the fitting procedure is very precise.
Therefore both the fitting and the dispensing of the lenses is done here in our office. If you have
any questions concerning contact lenses, please ask.

Presently, we accept both Medicare and Medicaid assignment on all approved diagnosis and procedures.
We also accept most insurances, many HMO and PPOs.
Payment is expected at the time service unless we have previously approved another plan.
Your payment at the time of service is provided helps contain costs by avoiding extra billings and
financial record keeping.

It is our standard office policy to provide an estimate of charges when requested by patients. Please
ask the receptionist when you make an appointment or speak to Dr. Croley when you receive medical services.

Our office goal is to provide the necessary medical care our patients require without the burden of financial worry.
Should you need a major medical procedure, we urge you to discuss the cost of this service with Dr. Croley.
Our office personnel will help you make the financial arrangements.

To assist you in insurance reimbursement, our office provides trained insurance personnel to help file
your claim. As an added service to our Medicare patients, we will file your claim directly via our in-office computer.
Please understand that the insurance contract is an agreement between you and your insurance company.
We cannot accept responsibility for collecting your claim or for negotiating a settlement on a disputed claim.
We do, however, monitor insurance reimbursement changes and will be glad to answer any question you might have.
For those patients who require surgery, our in-office insurance specialist will file your claim.
We need to mention that the term "usual and customary" used by the insurance industry is based on survey
analysis usually several years old. This does not provide adequate consideration for today's cost or for
those patients who require treatment that is special or extraordinary.
